Acknowledgement
Acknowledge and fully understand that I, or that who I am the guardian of, will be engaging in a contact activity that could result in serious injury, including permanent disability or death, severe social & economic losses due to not only my own actions, inactivity or negligence, but also the action, inactivity or negligence of others, the rules of the sport of Judo, conditions of the premises or of any equipment used. Further, I acknowledge that there may be other risks not known to me or not reasonably foreseeable at this time.
Safety
Agree that prior to participating in any self-defense, jujitsu or Judo activity, I have a right and duty to inspect the mats, equipment, and facilities to be used, and if I believe anything is unsafe or beyond my capabilities, I will immediately advise my instructor, coach or supervisor of such condition(s), and I will refuse to participate.
Rules and Etiquette
Please review and complete the following form to accept our dojo rules and etiquette.
Safety and Hygiene
- No food, drink or chewing gum is allowed on the mat. Water bottles can be kept at mat side.
- Fingernails and toenails are to be kept clean and trimmed to a reasonable length.
- Participants should maintain good physical hygiene.
- Proper Judo Uniforms should be worn
- While training a t-shirt or compression shirt (also called a rash-guard) must be worn under the jacket. Athletic shorts, tights, yoga pants or compression shorts are recommended to be worn under the pants.
- The Judo uniform, also called a Judogi or a Gi, should be cleaned before training, this includes any clothing worn under the gi.
- Injuries should be reported and tended to immediately. Any blood should be cleaned immediately.
- No student will be allowed to train while under the effects of any drugs or alcohol. If a participant is suspected of being under the influence of any mood-altering substances, they will be removed from practice.
- Any metallic jewelry should be removed before practice.
- If the participant has long hair, it should be put up and kept tidy.
Dojo Etiquette
- Be on time! Classes start promptly on time. Students who are late should wait to be invited onto the mat, bow to the instructors, and greet the rest of the class. If a judoka knows they will be late, let the instructor know.
- Being consistently late can be considered a sign of disrespect or lack of commitment.
- Strive to attend as many classes as possible. Repetition and consistency are the best ways to learn Judo.
- Black Belts should be addressed as Sensei.
- Non-Black Belt Instructions should be addressed as Coach.
- Raise your hand, ask questions!
- Red River Judo does not discriminate based on race, creed, color, ethnicity, national origin, religion, sex, sexual orientation, gender expression, age, height, weight, physical or mental ability, veteran status, military obligations, and marital status.
- If at any time a person experiences any issues with discrimination while at Red River Judo, at a Red River Judo related function, or through a Red River Judo managed social media platform, we ask that the person report this immediately to a member of the staff and we will do our absolute best to rectify the situation.
I acknowledge that I, or that who I am the guardian of, will follow all rules and etiquettes of training Judo.
I acknowledge that I, or that who I am the guardian of, understands the importance of following these rules and etiquettes for your/their own safety and the safety of others.
Release
Release, discharge and covenant not to sue Red River Judo, Red River TaeKwonDo, the United States Judo Association, the United States Judo Federation, and the United States Judo, Inc. (dba USA Judo), their affiliated clubs, their respective administrators, owners, directors, agents, coaches, & other employees or volunteers of such organization, event officials, medical personnel, other participants, their parents, guardian(s), supervisors and coaches, sponsoring agencies, sponsors, advertisers and, if applicable, owners, lessors and lessees of premises used to conduct the events all of whom are hereinafter referred to as "releaseeā, from any and all claims, demands, losses or damages on account of injury, including permanent disability and death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releasee or otherwise.
Release of Video and Still photography
By your attendance at Red River Judo, you are granting your permission for you and/or the person you are the guardian of, to be filmed, videotaped, audio taped, or photographed and are granting full use of your likeness, voice, and words without compensation.
Right of Refusal
Agree that the instructors at Red River Judo can refuse training to anyone for any reason and may permanently remove me from participating in classes at Red River Judo.
Card on file
I authorize this business to keep my credit card on file for any future transactions until I request a cancellation.
I understand that the terms of this authorization will remain in effect until I request that they be terminated. I acknowledge that I am responsible for making this request in writing.
Understanding and Assumption of Risk
I, the undersigned, hereby acknowledge and fully understand the risks involved, including the possibility of injury, permanent disability, or death, and I accept full responsibility for any damages that may result from such risks.
As the parent(s) or legal guardian(s) of a minor participant (age 17 and below), I affirmatively state that I have provided the necessary instructions to the minor regarding the aforementioned warnings, conditions, and their implications. I consent to the minor's participation in the activity.
I HAVE READ THE ABOVE WAIVER & RELEASE, UNDERSTAND THAT BY SIGNING IT I AM WAIVING SIGNIFICANT RIGHTS, AND, BEING AWARE OF THIS, VOLUNTARILY SIGN IT. I ACKNOWLEDGE THAT I AM PARTICIPATING IN THE ACTIVITY WITH FULL KNOWLEDGE OF THE RISKS AND CONDITIONS INVOLVED, AND I DO SO OF MY OWN FREE WILL.
